First, if you believe your loved one's life is in immediate danger, call 911 right away, or remove them from the facility if deemed necessary. If there is no immediate danger, and your loved one still retains the cognitive ability to talk about the situation, speak with them, and attempt to get an accurate picture of the events that led to the problem.

Second, know your loved one's rights. If you suspect that neglect or abuse is occurring, have your loved one (or their power of attorney) request a copy of all the records from the facility regarding their stay there. If you have a reasonable belief that abuse or neglect is occurring, report the facility to the Illinois Department of Public Health at 1-800-252-4343.
